Insurance Policies

We participate with many insurance companies and are a preferred provider for:

  • Medicare
  • BC/BS
  • Ameriben
  • United Healthcare Commercial
  • Workers' Compensation (Only taking Workers' Compensation of Arizona at  this time.)
  • U.S. Dept. of Labor
  • UMR

Effective February 01, 2018 we will no longer accept AHCCCS/ UHC Community Plan.

If your insurance covers your treatment, you will receive the benefit of reduced personal costs. Insurance policies vary, so we will review your insurance to determine the appropriate course of action.

Once treatment has started, we will file your claims as a courtesy to you. To help us with your insurance filing, please complete the insurance questions on our questionnaire.

Your estimated patient portion and all co-pay must be paid at the time of service.

Regarding Automobile Insurance: We do not accept auto liens. If you need to be seen as a result of an automobile accident, please let us know, and we will explain your options.

Please remember that medical insurance is a benefit for you or your child. If, during the course of treatment, your benefits change, the financially responsible party is accountable for all charges.
