We are open to serve you, answer questions or schedule an appointment during the following hours:
Monday – Thursday: 9:00 a.m. – 4:30 p.m.
Friday: 9:00 a.m. – 2:00 p.m.
Your scheduled appointment time has been reserved specifically for you, and we request a 24-hour notice if you need to cancel your appointment. We ask for this notice because we have a list of waiting patients that can be seen in your appointment slot. If you miss your appointment without giving adequate notice, your account will be subject to a $50 no-show fee. You will need to pay this $50 charge before we see you again. We are aware that unforeseen events sometimes require missing an appointment, and we appreciate your cooperation. We also ask that Spanish speaking patients bring a translator to their visit, or they may be rescheduled.
If you have had images taken recently, please bring the disk and the report to your appointment. Since we have a different system than the hospital, we might not be able to read or view what was done. In this instance, you may be rescheduled.
Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts.
Your estimated patient portion and all co-pay must be paid at the time of service.
As a courtesy to our patients, we will bill insurance companies for services.
We do not accept auto liens. If you need to be seen as a result of an automobile accident, please let us know, and we will explain your options. Failure to disclose this information up front could result in termination from this practice.
